About The Role:
The PACE program is a center for patients 55+ in the community to come for basic healthcare needs from providers as well as have the opportunity to socialize with other patients in the area. Activities, games, and food are provided to members. Providers have a very low patient caseload and often only about 4 hours of their day is dedicated to patient appointments.
Job Description:
We are seeking a dedicated and experienced Nurse Practitioner to join the team. The Nurse Practitioner will be responsible for providing primary care services to senior patients, including conducting assessments, diagnosing illnesses, developing treatment plans, and collaborating with the interdisciplinary team to provide comprehensive care.
Qualifications:
- Valid NJ Nurse Practitioner license
- DEA and prescriptive authority
- Minimum of 2 years of Primary Care experience
- Strong interpersonal and communication skills
- Ability to work effectively in a team environment
