POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Care Coordination / Discharge Planning

MINIMUM REQUIRED QUALIFICATIONS - 1+ years Case Management experience

Travel experience

RN license

PREFERRED QUALIFICATIONS - HCA Case Management experience

Midas, Interqual, Meditech experience

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - M-F 8am-5pm / 40hrs per week

SYSTEMS - Midas, Meditech, Interqual, Care Port

START DATE - 12/30/2024
