The Case Management department at Jamaica Hospital currently seeks RN Case Managers for immediate full-time career opportunities.
Responsibilities:
- Responsible for supplying managed care insurers with documentation supporting acute inpatient hospitalization for patients on their assigned units.
- Identify the need for post discharge services as well as identify potential barriers for a safe and timely discharge.
- Collaborate with all members of the multi-disciplinary team to provide safe and timely discharge of patients.
- Associate degree in Nursing required, Bachelor of Science degree in Nursing preferred
- NYS Registered Nurse license required.
- PRI and SCREEN certification required (may be obtained following hiring).
- Propensity for compassion and ability to relate to patients with various needs.
- Great communication skills and ability to articulate care plans to patients and their families.
- Excellent organization and record-keeping skills, and the ability to multitask.
- Sound critical thinking, problem-solving and interpersonal skills.
- Strong computer literacy to maintain and manage case records.
