Position Summary

Under minimal supervision and in a professional and friendly manner, perform assigned job duties to the highest level of customer service and assistance to both internal and external customers of Memorial Care Medical Foundations Inpatient/Specialty Services unit .

Essential Functions and Responsibilities of the Job

  1. On-site, telephonic or faxed clinical reviews (admission and concurrent) for MCMF patients in identified settings.
  2. Document appropriately and per regulatory requirements within aligned CM system.
  3. Provide discharge planning per assignment and in accordance with Health Plan and DOFR
  4. Support after hours duties for inpatient workflow if assigned.
  5. Be at work and be on time.
  6. Follow company policies, procedures, and directives.
  7. Interact in a positive and constructive manner.
  8. Prioritize and multitask.
  9. Monitor and implement all regulatory changes impacting care.
  10. Adhere to all company policies, procedures, and directives.
  11. Interact in a positive, professional, and constructive manner.


*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Qualifications

Minimum Requirements

Qualifications/Work Experience:

  • Current California Registered Nurse license (RN) required
  • Associate degree (AA) in nursing or equivalent from a two-year college or technical school required
Education/Licensure/Certification:
  • Bachelor of Science in Nursing, Social Worker or equivalent preferred
  • CCM preferred
