Role & Impact:
The Child Welfare Case Manager will provide comprehensive case management services to LSF clients and families, fostering meaningful relationships and embracing the agency's core values.
Key Responsibilities:
- Accept assigned cases promptly and meet with children and parents in their primary residences within the specified time frames.
- Collaborate with families and support stakeholders to develop targeted case plans that address the unique strengths and needs of each family.
- Conduct regular home visits to ensure child safety and wellbeing, maintaining consistent and thorough communication with all parties involved.
- Document all case-related activities accurately and timely in the Florida Safe Families Network database.
- Provide timely service referrals and maintain strong relationships with community resource providers.
- Participate actively in court proceedings, ensuring all required documentation is completed correctly and on schedule.
- Conduct ongoing assessments and adjust safety plans as necessary.
- Facilitate connections between children, parents, and family members by organizing visits and communication.
- Transport children as required and assist with Independent Living functions for eligible youth.
- Attend training, meetings, and other professional development opportunities as mandated.
- Work collaboratively with colleagues across various departments to ensure a seamless approach to child welfare.
- Prioritize and efficiently manage time to meet all deadlines and regulations.
- Adhere to all confidentiality rules and practices while treating all individuals with respect and dignity.
- Attend all required trainings to maintain certifications and gain essential skills.
Qualifications:
Physical Requirements:
- Valid Florida Driver's License and Insurance.
- Ability to travel locally and conduct professional duties, trainings, and conferences.
- Capacity to operate a computer and complete written correspondence effectively.
- Adaptability to irregular hours and flexibility regarding on-call duties.
- Certified in First Aid/CPR and crisis intervention.
Educational Background:
- Minimum of one year of relevant experience; certification is required within one year.
- Bachelor's degree in a Human Services field is required, with a preference for degrees in Social Work.
Skills & Abilities:
- Exceptional written and verbal communication skills.
- Proficiency in Microsoft Office programs, particularly Word and Excel.
- Strong understanding of case ownership and the needs of children and families.
- Sensitivity to the cultural and socioeconomic characteristics of the service population.
