Role & Impact:

The Child Welfare Case Manager will provide comprehensive case management services to LSF clients and families, fostering meaningful relationships and embracing the agency's core values.

Key Responsibilities:

  • Accept assigned cases promptly and meet with children and parents in their primary residences within the specified time frames.
  • Collaborate with families and support stakeholders to develop targeted case plans that address the unique strengths and needs of each family.
  • Conduct regular home visits to ensure child safety and wellbeing, maintaining consistent and thorough communication with all parties involved.
  • Document all case-related activities accurately and timely in the Florida Safe Families Network database.
  • Provide timely service referrals and maintain strong relationships with community resource providers.
  • Participate actively in court proceedings, ensuring all required documentation is completed correctly and on schedule.
  • Conduct ongoing assessments and adjust safety plans as necessary.
  • Facilitate connections between children, parents, and family members by organizing visits and communication.
  • Transport children as required and assist with Independent Living functions for eligible youth.
  • Attend training, meetings, and other professional development opportunities as mandated.
  • Work collaboratively with colleagues across various departments to ensure a seamless approach to child welfare.
  • Prioritize and efficiently manage time to meet all deadlines and regulations.
  • Adhere to all confidentiality rules and practices while treating all individuals with respect and dignity.
  • Attend all required trainings to maintain certifications and gain essential skills.

Qualifications:

Physical Requirements:

  • Valid Florida Driver's License and Insurance.
  • Ability to travel locally and conduct professional duties, trainings, and conferences.
  • Capacity to operate a computer and complete written correspondence effectively.
  • Adaptability to irregular hours and flexibility regarding on-call duties.
  • Certified in First Aid/CPR and crisis intervention.

Educational Background:

  • Minimum of one year of relevant experience; certification is required within one year.
  • Bachelor's degree in a Human Services field is required, with a preference for degrees in Social Work.

Skills & Abilities:

  • Exceptional written and verbal communication skills.
  • Proficiency in Microsoft Office programs, particularly Word and Excel.
  • Strong understanding of case ownership and the needs of children and families.
  • Sensitivity to the cultural and socioeconomic characteristics of the service population.
