Phoenix, AZ (Osborne School District) Occupational Therapist Grades K-8 24-25' School year Start date would most likely be August 1st or 2nd Caseload - approx. 70 students 8 hours (32 hours/ 4 days per week) $62hr Travel: Yes, multiple schools. Small District, so schools are not far from each other More details to come (additional information and questions will be answered during interview) The Occupational Therapist is responsible for performingclient evaluations, developing and providing occupational therapy services, anddocumenting services in accordance with the plan of care developed for eachpatient. Essential Duties and Responsibilities: Provides skilled occupational therapy services/interventionsin accordance with physician orders Reevaluates client for rehabilitation needs, and works incollaboration with the physician and other members of the healthcare team tomodify the plan of care as appropriate Assesses patient needs, plans for, evaluates and modifiescare to meet goals of Occupational Therapy Interventions Collaborates with all disciplines to plan and evaluate teamgoals for each patient Instructs patient/family/caregivers on patient's totaloccupational therapy program Advises, consults and instructs members of themulti-disciplinary team in specific occupational therapy modalities Observes and reports patient progress and/or changes inpatient status to physician and appropriate multi-disciplinary team members Refers individual to other disciplines as deemed appropriateand as approved by the physician Teaches new skills or retrains clients in once-familiardaily activities that have become difficult or unfamiliar due to illness orinjury Establishes equipment needs to facilitate independent living Instructs individual and/or family/caregiver in the use ofvarious types of adaptive equipment to improve function; ensures equipment isin good working order Trains individual and/or family/caregiver in use ofprosthetic and/or orthotic devices Performs work capacity evaluations and may refer individualto specialized vocational training in accordance with policy and regulations Provides accurate and timely documentation according to theplan of care Participates in care conferences or other meetings asrequired to ensure coordination of home health services (if applicable) Prepares and submits clinical progress-summary based on theprogress of goals Provides in-services/education programs for variousdisciplines as needed Participates in peer consultation process Supervises Occupational Therapy Assistants according tocompany policies/procedures and state regulations Participates in discharge planning when applicable Complies with applicable local, state, and federal laws andregulations regarding professional standards and practice standards May fabricate splints and instruct individual and/orfamily/caregiver in the use, application and benefits Performs other duties as assigned/necessary Minimum Requirements: Current licensure as an Occupational Therapist in the Stateof practice Must be a graduate of an accredited school or program ofOccupational Therapy One (1) year of prior professional Occupational Therapyexperience preferred Complies with all relevant professional standards ofpractice Participation and completion of Amergis' Competency programwhen applicable Current CPR if applicable TB Questionnaire, PPD or chest x-ray if applicable Current Health certificate (per contract or stateregulation) Must meet all federal, state and local requirements Successful completion of new hire training as applicable tojob site Understand patient confidentiality and HIPAArequirements Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required Computer proficiency required Must be at least 18 years of age Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs *Benefit eligibility is dependent on employment status.
