Position Summary:

We are seeking a compassionate and highly skilled Outpatient Nurse Practitioner to join our dynamic Comprehensive Transplant Center. This position will primarily involve outpatient clinic work, providing high-level care to heart transplant patients and collaborating closely with a multidisciplinary team. The ideal candidate will have a strong background in cardiology, transplant medicine, or advanced heart failure, as well as the ability to work effectively in a fast-paced, patient-centered environment.

Key Responsibilities:

  • Provide comprehensive outpatient care to heart transplant patients, including pre-transplant evaluations, post-transplant follow-up, and long-term management.
  • Perform clinical assessments, including physical exams, patient histories, lab work reviews, and diagnostics.
  • Develop, implement, and manage individualized care plans for heart transplant patients.
  • Monitor patient progress and adjust treatment protocols as necessary in collaboration with attending physicians, surgeons, and the transplant team.
  • Educate patients and families on transplant procedures, medication regimens, and lifestyle modifications.
  • Assist in coordinating the care of patients throughout their transplant journey.
  • Support heart transplant and heart failure clinic operations and maintain accurate, timely documentation in the electronic health record (EHR).
  • Provide mentorship and guidance to nursing staff and collaborate with other healthcare professionals in patient management.
  • Participate in call rotations, supporting the on-call care for heart transplant patients as needed.

Qualifications:

  • Nurse Practitioner (NP) certification in Maryland (or eligibility to obtain).
  • Master's or Doctoral degree in Nursing from an accredited program.
  • Previous experience in cardiology, transplant medicine, or advanced heart failure care is highly preferred.
  • Strong clinical assessment skills and the ability to develop and execute comprehensive care plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Ability to manage a high-volume patient caseload while maintaining excellent patient care standards.
  • Ability to work a flexible schedule, including evenings and weekends, to support on-call responsibilities.

Schedule:

  • Full-time position with a combination of outpatient clinic hours and on-call responsibilities.
  • Must be available to support call coverage for the Heart Transplant Program outside of regular business hours.
