Job Description

Job Summary:

The Respiratory Support Services Department will provide the services of a registered and licensed respiratory therapist whose primary job focus will be disease management for those patients suffering from chronic respiratory disease. The Respiratory Disease Navigator will work closely with Nursing Service, Physicians and other Ancillary Departments to develop and initiate plans of care and post-discharge services that promote the health and well-being of our respiratory patients.

Essential Job Duties:
  1. Perform intake assessment for patients admitted with chronic Respiratory illnesses.
  2. Conduct an interview and history.
  3. Develop a care plan.
  4. Recommend diagnostic testing to direct treatment plan.
  5. Identify candidates for Pulmonary Rehabilitation.
  6. Recommend and obtain referrals for post discharge diagnostic testing.
  7. Complete medication reconciliation and make recommendations for discharge medication plan: • Develop a medication regime that best meets the patients' needs • Evaluate the effectiveness of the patients medications • Evaluate the patient's ability to use the prescribed method
  8. Consult with Social Service and Case Management personnel to procure assistance for access to prescribed home regime.
  9. Perform education: • Disease specific • Disease self-management • Medication regime • Infection prevention • Respiratory therapy adjuncts • Nutrition • Exercise • Tobacco cessation • Other modalities or topics as necessary.
  10. Document course of action.
  11. Complete post-discharge follow-up with patients as necessary.
Secondary Job Duties That May be Reassigned:
  1. Perform Pulmonary Rehabilitation and other Respiratory Support Service (RSS) department activities in the absence of the other full time RSS therapist.
  2. Conduct tobacco cessation interventions and cessation education courses.
  3. Remains proficient in all duties of a Registered Respiratory Therapist I.
  4. Completes other duties and special projects as assigned.


Qualifications

Job Specifications:
  • Education: Minimum of an Associate Degree in related field. Bachelor's degree preferred.
  • Experience: A minimum of five (5) years of experience in the provision of Respiratory Care services. A background or training in wellness or disease management is desirable.
  • License/Credentials: Registered Respiratory Therapist (RRT) by the NBRC, Respiratory Care Practitioner license through the IPLA, Certified Asthma Educator (AEC)
