Description

The Assertive Community Treatment (ACT) SOAR/Housing Specialist position works in a multidisciplinary team to provide intensive case management services to persons with severe mental illness who are considered most at risk of unsuccessful independent living and subject to institutional services, care, and treatment. The ACT SOAR/Housing Specialist will assist these individuals in obtaining Social Security disability benefits, and maintaining housing through a strengths-based, recovery-oriented approach.

Essential Functions

  • Attends ACT, SOAR, and Housing team meetings to review referrals, address issues and report on client activities and progress.
  • Provides direct case management services to adults through ACT, SOAR, and Housing First models.
  • Collects and reports data in compliance with ACT, SOAR, and Housing models.
  • Assists individuals in the completion and submission of Federal Entitlement reviews from Social Security Administration (SSA) and KDHE's KanCare Title 19 Disabled Medicaid program.
  • Assists individuals in informing SSA and KDHE of physical address change, current phone numbers, emergency contacts, etc. to avoid disconnection of federal benefits.
  • Develops knowledge of community resources and how to access these services for the benefit of clients.
  • Collaborates with government agencies, community organizations, and property owners to build working partnerships and housing solutions.
  • Assess housing barriers of ACT consumers to determine housing and service needs to increase overall housing stability.
  • Provides mediation and advocacy with landlords on the participants behalf to develop a workable plan to obtain and maintain housing.
  • Ensures individuals understand their rights and responsibilities as tenants
  • Assists with crisis planning and provides crisis intervention services when necessary.
  • Provides back-up services to ACT, SOAR, and Housing teams as necessary.
  • Establishes and maintains a positive working relationship and professional demeanor with clients, external community partners, agency staff members, and visitors.
  • Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and CrossWinds standards and requirements. Submits daily, progress notes, client status reports, and all other related services, documentation and paperwork in a timely manner and acceptable format
  • Attends professional workshops and seminars in order to maintain and improve professional skills and meet licensure/certification requirements.
Work Environment

This job operates in both a community and a professional office setting. This position primarily works with clients out in the community, in their homes, in offices of other providers, and within the residential facilities. This role uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.

The employee must occasionally lift or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Language skills are required, including the ability to read, write, comprehend, and speak the English language. This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.

Position Type/Expected Hours of Work

This is a full-time position, 40 hours a week. Rotating weekend shift and on-call responsibilities are required as necessary.

Travel

Regular travel within Lyon to provide case management services and may include surrounding counties as needed.

Qualifications

Required Education and Experience
  • Associate Degree or 2 years' experience in the health and human services field.
Preferred Education and Experience
  • Bachelor's Degree in psychology, social work, counseling, or related field.
  • One year of experience working with SPMI population.
  • One year of experience in a specialty field.
License or Certification
  • Complete Case Management Training and provide training certificate to agency within six months of hire.
  • Complete training in one or more specialty fields.


Additional Eligibility Qualifications

Ability to work with persons with severe and persistent mental illness; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; ability to work independently and as a member of a multidisciplinary therapeutic team; possess prior computer experience in Microsoft Office programs and basic computer fluency.
