POSITION SUMMARY: We are seeking a dedicated Physical Therapist to provide outpatient rehabilitation services in an in-home setting specifically catering to the older adult population. The work environment could include patients' private residences or independent living facilities. This role allows Physical Therapists to evaluate and treat patients in the most functional environment—their home. As a Physical Therapist, you will assess needs, develop and execute personalized physical therapy programs, and identify when to make referrals to other disciplines to meet patients' comprehensive needs.

RESPONSIBILITIES/ACCOUNTABILITIES:

  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in compliance with state licensure and professional standards.
  • Ensure adherence to treatment schedules and timely clinical and billing documentation.
  • Participate in patient care discussions and contribute to meetings regarding staffing and care strategies.
  • Make appropriate referrals to address the comprehensive needs of patients.
  • Collaborate with a multidisciplinary rehabilitation team to determine the best therapeutic approaches for each patient.
  • Customize therapy programs based on individual patient needs.
  • Maintain a safe environment and address fire and safety hazards.
  • Order necessary supplies and equipment.
  • Ensure equipment is clean, maintained, and report repair needs.
  • Prioritize patient service to guarantee the highest quality of care in a compassionate setting.
  • Conduct screenings, examinations, and evaluations, including patient history and system reviews, to establish diagnoses and treatment plans.
  • Implement a variety of therapeutic techniques, including but not limited to: therapeutic exercise, functional training, manual therapy, health and wellness promotion, chronic disease education, and falls assessment.
  • Design and apply adaptive devices, educating patients on their use.
  • Select activities that are suitable for each patient's capabilities and interests.
  • Instruct patients in mobility techniques and assess their progress.
  • Perform routine re-examinations as needed to adjust treatment plans.
  • Incorporate health and wellness programming into treatment plans.
  • Conduct assessments for home and community mobility, providing tailored recommendations.
  • Fulfill other duties as assigned.

QUALIFICATIONS:

  • Valid Physical Therapy license in the state(s) of practice or proof of eligibility.
  • Master's degree in Physical Therapy or a related field.
  • Bachelor's degree in Physical Therapy plus five years of progressive experience may also be acceptable.
  • Graduates with foreign Physical Therapy degrees must have their credentials evaluated for equivalence.
  • Valid driver's license or reliable independent transportation to patient residences.
