JOB DESCRIPTION SUMMARY

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse, in a manner consistent with standards of practice. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

POSITION QUALIFICATIONS
  • Licensure/Certification/Registration: Currently licensed in Louisiana as a Licensed Practical Nurse (LPN).
  • Education: Graduate of an accredited nursing school.
  • Experience: Three years of full time experience as an LPN; Hospice experience preferred.
  • Knowledge of hospice philosophy preferred.
  • Good oral, written and organizational skills.
  • Computer literate and familiar with Microsoft Office software.
  • Possesses current Louisiana Driver's License, Automobile Liability Coverage and CPR cert.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
  • Provides direct patient care as defined in State Nurse Practice Act.
  • Implements current nursing practice following a comprehensive assessment and the plan of care.
  • Observe, record, and report to the registered nurse or Nursing Coordinator on the general physical and mental condition of the patient.
  • Administer prescribed medications and treatments as permitted by the State or Local regulations.
  • Receive orders from the physician and follow those that are within the realm of practice for an LPN and within the standards of hospice practice.
  • Ensure quality and safe delivery of hospice services to patients/families/caregivers.
  • Provide accurate and timely documentation of patient services to reflect the plan of care and Hospice policies and procedures.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary team. Examples include hospice aide, OT, PT, MSW, ST, Dietitian or Clinical Coordinator.
  • Uses equipment and supplies effectively and efficiently.
  • Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.
  • Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities.

WORKING CONDITIONS: Employee may be exposed to infections/contagious diseases. May have contact with patients under a variety of circumstances. May be near moving equipment.

Handles emergency crisis situations. Travel required.
