Qualifications:
- Social Work Degree
- Master's Degree in Social Work required
- Current State Licensure in Social Work
- LMSW or LCSW
- Minimum of one year's experience in health care or hospice setting
- Valid driver's license and proof of insurance required

What You'll Do:
- Perform psychosocial assessments of patients and families/caregivers
- Demonstrate exceptional service to patients, families, and co-workers

- Assist with applications for support programs and obtain community resources
- Serve as a liaison between patients and families/caregivers and community agencies
