Overview

St. Elizabeth Physicians is seeking a dedicated Nurse Practitioner (NP) to join our Ovation Practice, where we are committed to providing exceptional, patient-centered care. In this role, you will work as an extender of our physicians, collaborating closely with our healthcare team to deliver comprehensive services to our patients. Please note that this Nurse Practitioner will NOT carry their own patient panel.

Responsibilities

• Patient Care Management: Deliver high-quality, evidence-based patient care, focusing on health outcomes and patient satisfaction. Perform comprehensive assessments, develop and implement care plans, and monitor progress.
• Care-Based Model: Embrace a care-based model emphasizing holistic, coordinated, and preventive healthcare. Engage with patients to promote wellness, educate them on chronic disease management, and support healthy lifestyle choices.
• Team Collaboration: Work closely with physicians, specialists, and other healthcare providers to ensure seamless care transitions and optimize patient outcomes. Participate in regular team meetings and contribute to a culture of teamwork and collaboration.
• Patient Education: Provide education to patients and their families about medical conditions, treatment options, and preventive health measures. Advocate for patient needs and preferences in care decisions.
• Documentation and Compliance: Maintain accurate and timely documentation in electronic health records, ensuring compliance with all regulations and standards.

Qualifications

• Master's degree in nursing
• Current NP license in the state of Kentucky
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a team-oriented environment
• Passion for patient-centered care and community health
