Marin Post Acute in San Rafael, CA, is seeking an experienced LVN MDS Coordinator to join our dedicated team. If you have a strong background in MDS assessments and are committed to ensuring the highest standards of resident care, this is a fantastic opportunity for you. We offer competitive pay between $48-$55 per hour, depending on experience.

- Conduct and coordinate comprehensive MDS assessments in compliance with state and federal regulations.
- Develop and update individualized care plans based on assessment findings and interdisciplinary team input.
- Ensure accurate and timely documentation to support optimal reimbursement and regulatory compliance.
- Collaborate with nursing staff, therapists, and other healthcare professionals to deliver comprehensive care.
- Participate in care plan meetings and contribute to continuous improvement initiatives within the facility.
- Monitor and audit MDS processes to ensure quality outcomes and compliance with all relevant regulations.
- Current California LVN license.
- Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting.
- Strong understanding of MDS 3.0, care planning, and regulatory requirements.
- Excellent organizational skills and attention to detail.
- Strong communication and teamwork skills, with the ability to work collaboratively with a multidisciplinary team.
- Competitive hourly wage between $48-$55 per hour
