In coordination with the Medical Director, assumes primary responsibility for the care of all patients. Physician/NP availability is provided on a 24 hour/day, 7 day/week basis unless otherwise determined by the Medical Director.

Essential Duties and Responsibilities for a Physician:

  • Performs initial and ongoing patient assessments and documentation. Develops and initiates care plans to address medical, physical, spiritual, and psychosocial needs. Identifies, evaluates, and addresses disease progression and symptom management.
  • Integrates practice as part of an interdisciplinary team to provide optimum quality care.
  • Organizes and prioritizes assignments and work time.
  • Anticipates problems and modifies plans and procedures to provide solutions.
  • Participates in family conferences.
  • Performs patient, family, caregiver, and facility staff teaching in all given situations; provides on-going teaching and support to staff and volunteers.
  • Participates in medical education activities.
  • Documents to agency and accepted medical standards with timely completion, timely signatures, and content to support hospice stay in program and level of care.
  • Adheres to requirements of both HPCB and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organization focused (administrative).
  • Adheres to HIPPA regulations and policies; maintains the security of patient electronic/written records.
  • Participates in provider call schedule including periodic weekend, holiday, and overnight coverage, along with cross-coverage needs.

Qualifications for a Physician:

  • Education/Certification
    • MD or DO degree; licensed and currently registered in the State of New York to practice medicine.
  • Board certified or board eligible in a medical specialty.
  • Hospice and Palliative Medicine board certification or fellowship completion preferred.

  • Experience
  • Must have been actively engaged in the practice of medicine in the year prior to employment.
  • Hospice/palliative medicine experience preferred.
