The Health Collaborative, a non-profit agency that implements the Grow Healthy Together Pathways

Community HUB, is currently seeking Community Health Workers for Bexar County and surrounding

communities. These positions will be located in San Antonio but may serve residents in other

municipalities and counties surrounding Bexar County, Texas.

A community health worker (CHW) is a certified healthcare professional that provides:
• Cultural mediation between communities and the health care service system
• Culturally-appropriate and accessible health education and information
• Informal counseling and social support
• Advocacy for individual and community needs
• Building of individual and community capacity

The Pathways Community HUB is an evidence-based approach program that relies on

Community Health Workers (CHWs) to guide individual participants through the pathway steps

to mark their progress toward improved health and well-being.

The Grow Healthy Together Pathways Community HUB is a Tier 1 Certified HUB and therefore all

CHWs must adhere to the certification standards of the National Accrediting Agency, Pathways

Community HUB institute.

FULL CHW JOB DESCRIPTION

Qualifications and Requirements
• High School Diploma or high school equivalency certificate preferred. College Degree in

Social Sciences, Community/Public Health or related field is a plus
• Certified as a Community Health Worker by the Texas Department of Health and Human

Services. Experience in lieu of certification will be considered
• Proven leadership experience with public health and/or social services in a community

setting
• Understand and be part of the community being served
• Be experienced in community work, education, health care, or related field preferred
• Sensitivity and experience in working within diverse cultures
• Have ability to read, write, and speak both Spanish and English; fluency in spoken Spanish

required. Other languages are also of significant interest
• Possess Information Technology skills such as: Laptops, Smartphones, Internet/online

application systems, Microsoft Office, etc.
• Must demonstrate the ability to work independently or with little supervision
• Must be able to work in teams and embrace cross-sector collaboration
• Must demonstrate strong organizational and planning skills
• Ability to separate personal from professional and to establish boundaries with participants
• Must have reliable transportation to perform home/neighborhood visits
• Valid Texas driver's license with one year of driving experience and no traffic violations

Essential Duties
• Assesses eligibility and level of readiness for adults/families to enroll in Grow Healthy

Together Pathways HUB Program
• Provides basic family and individual facilitation; works with individuals to develop prioritized

pathways
• Coordinates and follows up on referrals to other community organizations that specialize in

areas prioritized by individuals (may include comprehensive and facilitated referrals as well

as application assistance and advocacy)
• Perform navigation work with minimal supervision. May be required to occasionally work

irregular hours, attend other job-related meetings and/or trainings, and perform other

duties as assigned
• Provides current and appropriate general health and wellness education throughout

interaction with participants as well as decreasing dependency through the teaching of selfadvocacy skills
• Provides input into the development and implementation of community programs created

in response to the identified needs
• Identifies and documents systems barriers in efforts to connect participants to health and

social services
• In certain circumstances, may interpret (in Spanish or another language) for Limited English

Proficient (LEP) participants
• Maintains participant-tracking system in the Pathways HUB Connect database, including

updated information on participant, demographics, eligibility and enrollment status, and

referrals made and followed up on
• Performs home or neighborhood visitations

All training and additional required certification will be provided to the CHW by The Health

Collaborative.

Successful CHW's are familiar with the communities they work with, and work in team environments

while building trust with their community members, local service organizations and healthcare

providers.
