Description

PRIMARY FUNCTION/ PURPOSE:

The function of the Nurse-Family Partnership (NFP) Nurse Home Visitor is to provide comprehensive community health nursing services to women and their families eligible for the NFP Program who reside in or around Bexar County. The Nurse Home Visitor is responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and The Children's Shelter. The Nurse Home Visitor reports to the NFP Nurse Supervisor and in his/her absence reports to the Vice President of Programs of The Children's Shelter.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Conducts in-person visits in accordance with the NFP Model Elements and NFP Visit-to-Visit guidelines.

• Develops therapeutic relationships, and utilizes concepts of reflection and motivational interviewing with women and their families in an in-person visiting environment.

• Assesses the physical, emotional, social, and environmental needs of women and their families as they relate to the NFP domains.

• Assists women and their families in establishing goals and outcomes.

• Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals.

• Consults and collaborates with other professionals involved in providing services to women and families.

• Formulates nursing diagnosis and evaluates client progress toward stated goals and NFP outcomes.

• Plans in-person visits in accordance with client goals and NFP outcomes.

• Manages a caseload of 25-30 clients.

• Supports the policies, procedures, guidelines, and standards of NFP, The Children's Shelter, and the state of Texas DFPS.

• Participates in the Community Advisory Board in support of NFP.

• Promotes public awareness of NFP.

• Develops and maintains community relationships to support client referrals.

• Completes all required NFP education and initial paid training at the National Service Office for Nurse-Family Partnership Program in Denver, Colorado (5 days).

• Attends and participates in ASQ and other training.

• Maintains clear, effective, open, and honest communication with both internal and external customers and colleagues.

• Enters accurate and timely documentation and services into DMCN and PIERS according to agency protocols and timeframes.

• Performs accurate data input to the NFP performance management system.

• Stays informed of current healthcare developments to provide safe, quality nurse home visiting services.

• Attends and completes required in-services and training.

• Performs other duties as assigned to ensure continuity of operations.

GENERAL EXPECTATIONS

• Contributes to building a child welfare system that is safe, secure, and stable; family-centered; connected to the community; which relies on evidence-based practice necessary to support contract services.

• Adheres to all policies and procedures of the agency and its accrediting and standard-setting bodies, such as Council on Accreditation (COA) and state licensing.

• Ensures all processes and tasks completed meet local, state, and/or federal compliance requirements.

• Maintains client confidentiality per The Children's Shelter policies and all applicable codes of ethics.

• Meets assigned deadlines and follows through on assignments.

• Learns, understands, supports, and demonstrates The Children's Shelter's mission, vision, and values.

• Accepts feedback, coaching, and supervision.

• Demonstrates the Trauma Informed Care principles and utilizes them with peers, clients, and external stakeholders.

• Understands and demonstrates the Praesidium best practices to ensure the safety of clients.

• Promotes teamwork within team, program, and agency.

• Promotes and demonstrates appropriate respect for cultural diversity among coworkers and clients.

• Attends and successfully completes mandatory trainings as defined in positions training matrix by the assigned deadline. Submits certifications of trainings to Training Department.

• Uses outside professional collaboration and intra-agency teamwork appropriate to the agency.

• Avoid all conflicts of interest, real or perceived.

• Attends work in accordance with agency leave policy.

Requirements

EDUCATION AND EXPERIENCE:

• Bachelor of Science in Nursing from a U.S.-accredited college or university is required.

• Licensure as a Registered Nurse in accordance with the state of Texas or possess a nursing multi-state license through the Nurse Licensure Compact.

• Current cardiopulmonary resuscitation certification (CPR).

• Fluency in English/Spanish is highly preferred.

• A minimum of two (2) years of recent experience in maternal/child health, public health, or home-visiting nursing is required.

• Must have and maintain a valid Texas driver's license and reliable transportation with proof of insurance.

KNOWLEDGE, SKILLS, AND ABILITIES:

• Strong computer skills, specifically in the use of Microsoft applications and client databases.

• Ability to enter client data into the DMCN and PEIRS.

• Ability to plan, organize, and coordinate work assignments independently.

• Ability to communicate effectively, clearly, and concisely through oral and written means.

• Ability to establish and maintain effective working relationships with others.

• Ability to understand and apply applicable rules, regulations, policies, and procedures.

• Background check, drug testing, and TB screening required post-offer prior to employment.

DESCRIPTION OF WORKING ENVIRONMENT:

This is a full-time position, scheduled for 40 hours a week with the need for a flexible schedule. Work environments of the NFP Nurse Home Visitor consist primarily of office and community activities, including home visitation and community outreach. Traveling to designated locations will be required on a regular basis.
