Summary The Emergency Nurse Practitioner is a skilled healthcare provider who utilizes critical judgment in the performance and direct management of acute/chronic illness and disease. Nurse Practitioner practice promotes wellness and prevents illness and injury. Emergency Department Scope of Practice (But not limited to): 1. Provides leadership & professionalism in an advanced practice role. 2. Provides medical screening in an emergency setting. 3. Medical decision making & differential diagnosis. Responsibilities Practice: Provides leadership in the application of the nursing process to client care, organizational processes and/or systems improving outcomes at the program or service level. Demonstrates knowledge and required safety skills in the following: emergency preparedness, infection control. hazardous material management, fire and equipment failure emergency management, and utility outage response procedures. Utilizes the eight core competencies of the High Performance Development Model (HPDM) in practice. Demonstrates adherence and competence related to Nursing Standard Operating Procedures, VASNHS Medical Center Memoranda, and National Directives. Participates in the care of patients seen in Emergency Department and Fast Track. Coordinates and oversees emergency staff in the delivery of complex including patient evaluation and treatment. Provides patient education to the patent and family. Coordinate and participate in implementation of clinical practice guidelines used within the Emergency Services or across departmental, divisional, or section lines. Demonstrates leadership in analyzing ethical issues and applies appropriate theoretical principles to the ethical decision-making process. Evaluates available options and selects a course of action which will result in the most favorable patient or organizational outcome. Demonstrates ability to work effectively with other healthcare providers in the identification, analysis, and resolution of healthcare issues specific to non-institutional care practice standards. Leads and participates on nursing or healthcare system committees, councils, and/or workgroups. Uses effective communication strategies with individuals and groups to facilitate decision-making, improve care delivery systems, and solve problems while promoting a positive work environment. Shares clinical/professional expertise with others within or external to the facility, through publication, consultation, presentations, or participation in professional organizations. Ethics: Provides leadership in identifying and addressing ethical issues that impact client and staff, including initiating and participating in ethics consultations. Analyzes ethical issues and appropriate theoretical principles Ensures application of institutional policy. Serves as a resource for the protection of patient/family rights. Resource Utilization: Manages program resources (financial. human, material or information) to facilitate safe, effective and efficient care. Analyzes resource utilization to assure safe, effective and appropriate patient care delivery. Develops and implements changes which influence the efficiency and quality of care Assesses impact of situations to predict potential outcomes for the section/service and/or nursing practice. Takes action to assure that outcomes are positive. Education/Career Development: Implements an educational plan to meet changing program or service needs for self and others. Maintains knowledge of current techniques, trends and professional issues. Performance: Uses professional standards of care and practice to evaluate programs and/or service activities. Accepts continuing responsibility for own role development. Examines and interprets standards of the profession. Quality of Care: Initiates and/or participates in interdisciplinary projects(s) which document improvement in organizational performance. Actively participates in medical center s APN Council, nursing and other committees integral to departmental and medical center needs. Research: Collaborates with and assists others within or across work groups to validate and/or improve practice (solve practice problems/issues) either on the basis of application of current research-based knowledge or the conduct of research This may include methods to evaluate the outcomes of practice changes (innovation/interventions) and reporting the results as appropriate. Qualifications Advanced Practice Nurse (Nurse Practitioner) Basic Requirements English Language Proficiency. In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment. License and Certification: License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared. NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (NP) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) II: Exception to Licensure for Graduate Nurse Technicians. Candidates who otherwise meet the basic requirements, but do not possess the required licensure and/or certification, may be appointed at the entry step of the Nurse II, as a Graduate Nurse Technician - Nurse Practitioner on a temporary appointment up to any period not-to-exceed one year and a day under the authority of 38 U.S.C. § 405(c)(2)(B). Scope: Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others Education and Experience Requirement: Meets basic education requirement for APN (NP). Nurse II is considered the entry grade level for APN (NPs); no experience is required. Dimension Requirement: APNs (NPs) at level II are expected to meet the following dimension requirements: Clinical Practice. Delivers evidence-based health care, demonstrating proficiency in the ability to manage a patient panel, population, or program. Outcome Focus. Utilizes research and evidence-based practice to evaluate the patient care delivery system within a patient panel or program, presents findings and contributes to changes that enhance the quality and timeliness of veteran/patient care. Leadership. Works effectively as a leader within a team/unit/group and seeks interdisciplinary collaboration as appropriate to enhance patient care. Professional Development. Acquires knowledge and skills to develop expertise in area of practice. Professionalism/Collegiality. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Participates in interdisciplinary teams and processes that positively impact patient outcomes. The following Scope, Education and Experience, and Dimension criteria must be met to qualify for Advanced Practice Nurse (Nurse Practitioner) III: Scope: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Education and Experience Requirement: Meets basic education requirement for an APN (NP) AND one year of specialized APN (NP) experience equivalent to the Nurse II grade level. Dimension Requirement: APN (Nurse Practitioners) at grade III are expected to meet the following dimension requirements: Clinical Practice. Influences and models the delivery of evidence- based health care, demonstrating expertise in the ability to manage a patient panel, population, or program. Outcome Focus. Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations. Leadership. Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program. Professional Development. Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility. Professionalism/Collegiality. Initiates and leads interdisciplinary teams developing systems that positively impact outcomes at the panel, population, or program level. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Physical Requirements: Position requires heavy lifting (45 pounds and over), heavy carrying (45 pounds and over). straight pulling (4 hours), pushing (4 hours), use of fingers and both hands, walking and standing 8-12 hours, repeated bending 8 hours, both legs required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in other eye, depth perception, ability to distinguish shades of colors, hear (aide permitted) Transferring patients and objects required. The incumbent may be exposed to infected patients and contaminated materials and will be required to don protective clothing in isolation situations or operative/invasive procedures The incumbent may be exposed to patients who are combative secondary to delirium, dementia or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient and departmental needs Transporting patients in both wheelchairs and on stretchers may be necessary. The incumbent is expected to use proper preventive ergonomic techniques to perform duties. Education Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Colligate Nursing Education (CCNE) In cases of graduates of foreign schools of professional nursing possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript (Reference VA Handbook 5005, Appendix G6) EDUCATION/EXPERIENCE REQUIREMENTS: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. 1. Bachelors of Science Degree (BSN) and a Master's degree in Nursing from a program accredited by the National League of Nursing Accreditation Council (NLNAC) or Commission on Collegiate Nursing Education (CCNE) (Official/Unofficial transcripts MUST be submitted with application packet). 2. Minimum of 3 years of clinical experience as a Nurse Practitioner, and a minimum of 2 years of Emergency experience. Experience MUST be detailed as assumptions are a prohibited practice. 3. Excellent interpersonal skills and the ability to work independently as well as collaboratively with multiple services lines and disciplines. 4. Prescriptive authority requirements include evidence of national certification and completion of upper division education in pharmacology 5. Maintain full and current certification as a Nurse Practitioner from the American Nurse Credentialing Center (a subsidiary of the American Nurses Association), or another nationally recognized certifying body. (Copy of certification MUST be submitted). Grade Determination: Experience as a Nurse Practitioner will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry.
