Pembroke Pines, United States VITAS Healthcare Full time

WHO WE ARE

We are a nurse-founded hospice organization and a pioneer in end-of-life care since 1978. As caregivers, we advocate for our patients and their families, providing compassionate care at the bedside. We take care of each other, and we are proud to make a difference in the lives of those we serve.

We are VITAS.

WHAT YOU’LL DO

As an LPN in Continuous Care, you will play a vital role in providing comfort and support to hospice patients, ensuring they experience dignity and peace during their final journey. This position requires compassion, commitment, and a deep understanding of the importance of palliative care. You will be entrusted with the responsibility of delivering bedside care while fostering meaningful connections with both patients and their loved ones.

You will be a valued member of our team, not only as a caregiver but also as an educator to patients and families about hospice care. With a focus on comfort and symptom management, you will work autonomously, making critical decisions at the bedside. Your role will emphasize the importance of hospice care’s palliative nature, helping families embrace comfort-focused end-of-life care.

WHERE YOU’LL WORK

Each day will bring a variety of experiences as you provide care to patients in their homes, assisted living facilities, nursing homes, hospitals, or medical centers. Your schedule will adjust based on patient needs.

You will also participate in regular interdisciplinary team meetings (once a week), collaborating with social workers, physicians, chaplains, and other nurses to deliver comprehensive support to both patients and their families.

  • Schedule: 7:00 PM - 8:00 AM, with every other weekend.
  • Mileage Reimbursement: Yes, $0.67 per mile.
  • Benefits: Medical, Dental, Vision, 401K, HSA, FSA, Tuition Reimbursement, PTO.

WHAT’S EXPECTED FROM YOU

As a VITAS LPN, you will provide the exceptional, compassionate care that our patients and families deserve. In addition to holding a current LPN license, you will demonstrate empathy, reliability, and dedication to improving the quality of life for those at the end of their journey. You will be committed to our mission of delivering comfort and dignity to patients and their families at one of life’s most challenging times.
