Role Scope:

We are seeking a dynamic, service-oriented occupational therapist who is passionate about supporting elderly persons in their journey. The occupational therapist will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. In addition to clinical services, there is an opportunity to shape the company's occupational therapy program.

Participant Care:

  • Provide clinical occupational therapy evaluation and treatment on an acute, restorative, and maintenance level to program participants.
  • Participate in the Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination in a collaborative spirit.
  • Assess condition, create treatment plans, and discuss the implementation of therapy with participants and caregivers.
  • Instruct family members, caregivers, and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL), with documentation in medical record notes.
  • Make home visits as needed and document activities, including participant/caregiver response in the participant's chart.
  • Educate participants and family members in proper care and use of various supports and exercise programs, in addition to the use of DME.
  • Document therapy services provided and perform department record-keeping in keeping with documentation requirements.
Administration and Program Development:
  • Assist in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
  • Perform data collection, prepare reports, and maintain necessary participant statistics.
  • Attend all staff meetings and patient care conferences as scheduled.
  • Maintain the quality assurance, safety, environmental, and infection control programs.
  • Serve as a subject matter expert for the development of occupational therapy program standardization, evaluation, and team education.
  • Support quality programs and discipline-specific audits as a subject matter expert.
Required Qualifications:
  • Bachelor's degree in Occupational Therapy from an accredited program meeting AOTA & ACOTE standards.
  • National Certification by the American Occupational Therapy Association or eligibility for the National Registration Examination of the American Occupational Therapy Association.
  • Registration with the state of California.
  • A state-issued driver's license, personal transportation, and auto insurance as required by law.
  • A minimum of one year of experience working with the frail or elderly is required.
Preferred Qualifications:
  • 2+ years as a practicing OT.
  • Master's in OT (MOT) or Doctorate in OT (OTD).
  • Experience (or demonstrated interest) in clinical program development.
