ABOUT THE PROGRAM: Behavioral Health Homes

BHH provides medical care coordination & health and wellness support to identified existingCHR clients with significant behavioral health and chronic health needssuch as Diabetes, Obesity, Asthma/COPD, Coronary Artery Disease, Hypertension and Congestive Heart Failure.

DUTIES & RESPONSIBILITIES:

  • Assists Behavioral Health Home (BHH) enrollees identify and achieve their health and wellness goals.
  • Provides medical and behavioral care coordination and peer recovery support to clients with SPMI and/or chronic health conditions that meet BHH program criteria
  • Helps individuals recognize their capacity for recovery and resilience; uses maintenance interviewing concepts.
  • Provides client transportation as needed
  • Provides or supports BHH services including:
    • Comprehensive Care management
    • Care Coordination
    • Health Promotion
    • Comprehensive Transitional Care
    • Patient and Family Support
    • Referral to Community Support Services
  • Collaborates with a wide range of client supports including family members and community medical providers.
  • Provide skill-building and support for self management, self advocacy, health and well-being, and using community supports.
  • Works to reduce health care disparities whenever possible
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.


QUALIFICATIONS:

Education: High School diploma required.

Licensure/Certification/Registration: Valid driver’s license required.

Training: Recovery Support Specialist Certification from Advocacy Unlimited required at hire. Applicants due to complete certification within 6 months of hire will be considered.

COMPENSATION*:

$16.71/hour minimum, and up commensurate upon experience.

*Actual rates are determined at the time of offer and are based on relevant experience, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
