Home Health Physical Therapist- Modesto, CA (Offering Relocation Assistance)

Job Type: Full-time (Onsite)

Shifts: Standard Schedule, Rotating Weekends & On-Call

Job Summary:

Conduct thorough evaluations to assess patients' physical limitations, functional status, and rehabilitation needs. Utilize various assessment tools and techniques to determine baseline measures and track progress over time. Develop individualized treatment plans based on assessment findings, medical history, and patient goals. Maintain accurate and detailed patient records, complying with all legal and ethical standards. Collaborate with patients, families, and healthcare professionals to establish realistic rehabilitation objectives.

Qualification/ Requirements:
• A Master's Degree in Physical Therapy is required.
• Must have a Physical Therapist license in CA.
• Active CPR Certification preferred.
• 1 year of experience as Physical Therapist is required.
• Must be a licensed driver with an automobile that is insured.
