Summary:
• The Sterile Compounding Pharmacist is responsible and accountable for the performance and operation of the facility and personnel in the preparation of compounded sterile products (CSP) and all other functions pertaining to CSPs. The Sterile Compounding Pharmacist is responsible for ensuring that sterile preparations meet the clinical needs of patients according to quality, safety, and environmental control requirements, regulations, and standards in all phases of preparation, storage, transportation, and administration. In conjunction with pharmacy leadership, develops, implements, and oversees training of all compounders, personnel who have direct oversight of compounders, and personnel who perform restocking or cleaning and disinfection duties. Completes ongoing training and competency assessments for pharmacy personnel as outlined in departmental policies and procedures. The Sterile Compounding Pharmacist will comply with standards set by PPMC standard operating policy and procedure, U.S. Pharmacopeia Convention (USP), The Joint Commission accreditation standards, American Society of Health-System Pharmacists, the Pennsylvania Board of Pharmacy, and state and federal regulations.
Responsibilities:
• Oversees day-to-day operations in the sterile compounding area, ensuring the aseptic and accurate production of compounded sterile products (CSPs).
• Responsible for overall compliance with USP and , applicable federal and state laws and regulations and accreditation standards.
• In conjunction with pharmacy leadership, develops, implements, and oversees training of all compounders, personnel who have direct oversight of compounders, and personnel who perform restocking or cleaning and disinfection duties. Ensures that all persons who enter the sterile compounding area and/or handles CSPs complete training and demonstrate competency in maintaining the quality of the sterile compounding environment. • Ensures that each area where CSPs are prepared meets the classified air quality standard appropriate for the activities conducted in that area.
• Ensures that International Organization for Standardization (ISO) Class 5 areas are located, operated, maintained, monitored, and certified to have appropriate air quality.
• Reviews all certification and recertification records to ensure that the classified environments meet the minimum requirements outlined in USP and . Responsible for microbial environmental testing of air, surfaces, and personnel as outlined in USP standards.
• Responsible for oversight of personnel training and competency for those involved in sterile compounding and handling and preparing CSPs
• Ensures standard operating procedures and/or policies are fully implemented and that follow-up is carried out if deviations or problems are identified.
• Establish, monitor and document procedures for the handling and storage of CSPs
• Serves as an educational resource for pharmacy students, residents, and other health professionals.
• Participates in pharmacy operations, medication dispensing, and oversees pharmacy technician activities.
