Sapphire Health Services is currently seeking a dedicated Licensed Practical Nurse (LPN) to join our team and uphold our commitment to excellence.

JOB SUMMARY:

A Licensed Practical Nurse (LPN) in our Skilled Nursing Facility is responsible for delivering high-quality, person-centered care to residents, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory standards. This role involves assessing resident needs, implementing individualized care plans, administering medications and treatments, and providing leadership to nursing staff. The RN plays a crucial role in enhancing the health and quality of life for residents.

RESPONSIBILITIES:

  • Provide primary nursing care to residents with varying backgrounds and medical histories
  • Assists with new admissions
  • A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
  • Provide medical intervention as needed for emergent needs
  • Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc
  • As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
  • Implement and follow Primary Care Providers orders
  • Other Duties as Assigned
REQUIREMENTS:

Licensed Practical Nurse (LPN) Licensure in the State or practice
