Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

JOB SUMMARY

The Outreach Specialist is responsible for assisting identified participants in the community including connecting them to Medication Assisted Treatment (MAT) services, transportation and coordination of care needs. The State Opioid Response (SOR) Program provides support and services to individuals in the community that are struggling with opioid dependency or stimulant use disorder by creating increased access to services while focusing on the prevention from sustained use. An Outreach Specialist is an individual who is in recovery from AOD use and/or GMH for an appropriate period (greater than or equal to 1 year) and has at least 90 days experience as a Peer Support Specialist. The Outreach Specialist will complete outreach in the Page, Arizona community, work with agencies within law enforcement and the judicial system to identify those who need services and utilize resources to obtain safe and permanent housing/placement. Outreach Specialists will also assist in connecting participants to recovery services. The Outreach Specialist works as part of a multi-disciplinary team including specialty positions through the CBI continuum. The Outreach Specialist serves as a nexus, coordinating care with community providers and stakeholders.

ESSENTIAL JOB DUTIES OR RESPONSIBILITIES

  • Ensures that each individual under their care receives coordination with all parties currently identified as having an active role in their ongoing treatment and releases of information are completed.
  • Serves as a patient advocate ensuring the services the client needs are being received.
  • Navigates identified services or benefits and assists the individual in accessing these services.
  • Implement skills learned during on-the-job training and required competencies related to the program and ensuring that all required documentation is completed prior to the end of shift.
  • Protects everyone's confidentiality within the guidelines of established policy and process.
  • Adheres to established policy and process for this assignment.
  • Other duties as assigned by supervisory and/or administrative support.


Continuing Care
  • Assists the individual in the identification of needs for ongoing recovery, housing, and stabilization in the community.
  • Develops and collaborates with an extensive network of community based and RBHA resources. An Outreach Specialist must know how to access various social/behavioral health community-based services that can include but is not limited to food, clothing, supportive services, and outpatient crisis stabilization, and transition services. This can also include making necessary appointments, assisting with scheduling or arranging SMI and/or GMH evaluations, and outpatient/transitional stabilization services when applicable.
Individual Support
  • Provides emotional support to individuals who appear unsure or fearful, using calm reassurances, positive feedback, and sharing of personal recovery as appropriate.
  • Incorporates Motivational Interviewing techniques into interactions with individuals in program to empower them to achieve their goals of being housed and being permanently stable in the community.
  • Acts as a short-term recovery mentor, helping the patient develop and implement self-directed recovery plan, sharing experience, providing support, teaching self-management techniques, and reassurance.
  • Assists the individual in understanding the necessity for continued care and support through the program involvement, providing referrals when appropriate.


Outreach, Follow-Up, & Re-Engagement
  • Re-engages the individual if in need of additional services and coordinates such services as directed by the Housing and Community Integration Team.
  • Conducts outreach on the street and in the community to locate, engage, and build rapport with patients.
Transportation & Transfer
  • Provides transportation services to individuals to scheduled and relevant appointments identified on the individuals case plan.
  • Ensures that documentation is entered accurately and thoroughly in accordance to CBI Policies and Procedures.
Client Care & Safety
  • Documents the specific (who, what, where, when) in the appropriate Electronic Medical Record (EMR), of interactions with intoxicated/aggressive clients of what occurred in the event. Assists in sending Significant Events or Incident Reports when advised by Supervisor.
  • Observes universal precautions related to individuals presenting for crisis unit admission.
Customer Service
  • Treats individuals and any parties involved in their continuum of care with dignity and respect.
  • Provides community stakeholders (police, fire, probation, businesses, clergy, insurance companies, providers, RBHA's, concerned citizens, etc.) with the utmost respect and assist to the best of our ability in each case. Never saying "no" but always assisting each request within the context of the agency mission and purpose. Is evidenced by excelling in the area of customer service and not receiving a community complaint.
Documentation
  • Documents the specific interactions with individuals served being detailed and thorough reporting the facts.
  • Completes individual enrollments, assessments, and program requirements in the determined system.
  • Documents all activities daily in the appropriate EMR when appropriate.
  • Completes and deliver all Incident Reports and/or Significant Events, prior to the conclusion of the shift where the incident occurred, as specified in Community Bridges Policies and Procedures.
SUPERVISORY RESPONSIBILITIES

This position does not have supervisory responsibilities.

MINIMUM EDUCATION & EXPERIENCE REQUIRED
  • Highschool diploma or GED is required. Associates Degree (or higher) in a field related to Behavioral Health is preferred.
  • Must be in recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
  • Or equivalent combination of education and experience.
LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS
  • Current AZ Driver's License.
  • Clear 39-month Motor Vehicle Record.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures preferred.
OTHER SKILLS AND KNOWLEDGE REQUIRED
  • Able to adhere to all compliance regulations including, HIPAA; Fraud, Waste, and Abuse; Harassment; etc.
  • Able to read and comprehend information without difficulty.
  • Able to type 40/wpm.
  • Able to utilize Microsoft systems such as Outlook, Word, and Excel.

Applies knowledge and experience to analyze and assist with solving of moderate-to-complex issues involving some ambiguity. Able to develop solutions for improvement of clinical systems.
