CareGivers Home Health is seeking an upbeat and motivated intake coordinator to join our team.  The ideal candidate is extremely organized, has excellent people skills, and a strong work ethic.  We are looking for someone with a positive energy and can-do personality.

Responsibilities include guiding and assisting new home care patients through the onboarding process for receiving care.  This includes working with referral sources to obtain the required documentation to onboard the patient, communicating via phone or electronic means with referral sources and patients.
