Job Title: Registered Nurse (RN) Job Specialty: Home Care Job Duration: 13 weeks Shift: 8-hour days, Monday to Friday, with some on-call weeknights and rotation every 4th weekend. Guaranteed Hours: 40 hours per week Experience: At least 1 year as a Home Health RN License: State of Illinois RN license Certifications: Basic Life Support (BLS) Must-Have: - Patient assessment skills- Medication management- Wound care expertise- IV competency (PICC, peripheral, etc.) Job Description: - Conduct patient education and comprehensive head-to-toe assessments, including vital signs and breath sounds.- Perform skin and wound assessments, dressing changes, wound care, and manage Wound Vacs.- Provide care for PICC lines, including dressing and lab draws.- Conduct foley catheter and ostomy care.- Perform medication reconciliation and provide patient education on medications.- Utilize Epic for documentation and maintain communication with providers and care team.- Coordinate interdisciplinary care involving providers, therapy staff, social work, wound centers, and infusions as appropriate.- Use technology and equipment such as laptops, portable printers, and cell phones effectively. Additional Information: - Staff development includes 1:1 wound care, IV class, training, and online modules.- Clinicians are expected to see an average of 5 patients per day and complete documentation appropriately.- Orientation is approximately 5 to 7 days.
