Nurse Practitioner / Physician Assistant - Palliative Care Job Code: NP Reports To: Clinical Team Leader of Palliative Care Status: Exempt (Salaried) FT, PT, PRN: Full-time Shift: 40 hours per week - Monday - Friday 8:30AM-4:30PM On Call Rotation: Some nights and weekends phone call ( if included in contract) Location: Travels to visit patients in the following counties - Cherokee, Macon, Swain, Graham, Jackson, and Clay - mileage reimbursement included Are you looking for an incredibly REWARDING career? Essential Duties and Responsibilities: Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies Provide direct client care consultations, to include participation in patient and family counseling and management of symptoms related to life-limiting conditions in collaboration with the interdisciplinary team of physicians, nurses, social workers, and chaplains Take clinical call for palliative care clients Actively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of care Create and maintain accurate patient records Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation Serve as clinical resource for staff of Four Seasons, staff at facilities served, and providers in the community Maintain current medical education and appropriate Licensure Perform other duties as requested or assigned Knowledge, Skills and Abilities: Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation Knowledge of HIPAA privacy rules and regulations []{styl=""}
