Job Title: Infectious Disease Program Manager

Location: South Chicago 9119 S. Exchange Ave Chicago, IL 60617

Schedule: Monday - Friday 9:00am - 5:00pm

The Infectious Disease (ID) Program Manager will be responsible for overseeing the planning, implementation, and evaluation of programs aimed at preventing and controlling infectious diseases within a designated population or community. The ID Program Manager will work under the general guidance/supervision of the Chief Executive Officer and will be responsible for strategic management, coordination with healthcare professionals and stakeholders, and ensuring compliance with public health regulations and guidelines.

Essential Responsibilities:

Program Development and Implementation:
  • Design and develop comprehensive infectious disease prevention and control programs within CFHC Clinics.
  • Implement strategies to monitor and reduce the incidence of infectious diseases.
  • Develop educational materials and outreach initiatives to raise awareness and educate the public about infectious disease prevention.
Community Engagement:
  • Build and maintain partnerships with community organizations, healthcare providers, and government agencies.
  • Advocate for the needs of individuals living with HIV/AIDS.
Compliance and Reporting:
  • Oversee the collection, analysis, and interpretation of data related to infectious diseases.
  • Use data to identify trends, assess program effectiveness, and inform decision-making.
  • Prepare and present reports on program outcomes to stakeholders and funding agencies.
Coordination and Collaboration:
  • Work closely with healthcare providers, government agencies, community organizations, and other stakeholders to coordinate efforts and resources.
  • Foster partnerships and collaboration to enhance program reach and impact.
Resource Management and Quality Improvement:
  • Manage program budgets, resources, and personnel effectively.
  • Identify funding opportunities and assist in grant writing and fundraising efforts.
  • Supervise and support program staff, providing training and professional development as needed.
Qualifications:
  • Bachelor's degree in public health, Epidemiology, Nursing, or a related field (master's degree in public health, Nursing, Social Work, or a related field, preferred) or equivalent years of relevant experience.
  • Minimum of 5 years of experience in public health or infectious disease program management.
  • Experience with grant writing and fund development preferred.
  • Familiarity with electronic health records (EHR) systems, EPIC/Athena

Requirements

Education and Experience
  • Bachelor's degree in public health, Epidemiology, Nursing, or a related field (master's degree in public health, Nursing, Social Work, or a related field, preferred) or equivalent years of relevant experience.
  • Minimum of 5 years of experience in public health or infectious disease program management.
  • Experience with grant writing and fund development preferred.
  • Familiarity with electronic health records (EHR) systems, EPIC/Athena
  • Proficiency in payroll software and Microsoft Office Suite.
  • Excellent attention to detail and accuracy.
  • Strong analytical and problem-solving skills.
  • Ability to handle confidential information with discretion.
  • Excellent communication and interpersonal skills
