Position Overview:

The Per Diem Hospice House (HH) Registered Nurse (RN) passionately embraces the hospice philosophy, using teaching, assessment, and intervention skills to provide top-notch end-of-life care. They coordinate services, offering compassion and support to those in the terminal stage of illness and their families, following care plans and agency policies. The Hospice House RN also oversees LPNs, CNAs, and volunteers in the hospice house.

Minimum Qualifications:

1. Current unrestricted Washington state License as a professional Registered Nurse.

2. Two years of post-education RN experience.

3. In-depth knowledge of nursing care operations and administering medication.

4. Compassionate nature.

5. Excellent communication.

6. This position is designated as safety-sensitive and will require pre-employment drug testing, which includes cannabis testing in WA State

Per Diem Minimum Requirements

1. Available to work two (2) shifts per month.

2. Available to work evenings, nights, weekends, and holidays.

3. . Available to work two (2) holidays per twelve (12) month period.

PREFERRED POSITION QUALIFICATIONS

1. Hospice Experience, Oncology and/or Home Health Experience as a CNA, LPN or RN.

2. Bilingual.

CLINICAL STAFF POSITION QUALIFICATIONS

1. As required per position/credentialing; successful completion of Blood Bourne Pathogens Training.

2. Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS).

3. Washington State Food Worker Card issued by the Department of Health

Requirements

RESPONSIBILITES AND ESSENTIAL FUNCTIONS

1. Accept responsibility of care for patients admitted to or transferred into the Hospice House setting. A. Conduct an initial assessment of patient and family needs, including physical, emotional, social, and spiritual factors. B. Initiate and update the individualized plan of care in collaboration with the IDT. C. Accomplish nursing assessments of all the unit's patients during the shift according to policy. D. Administer medications and treatments to patients.

2. Request additional assessments and services from other disciplines on the team as needed.

3. Assess patient/family knowledge of disease and care and determine deficiencies in that knowledge.

4. Evaluate the patient/family's ability to learn and follow instructions.

5. Provide direct nursing care to patients to achieve the highest level of comfort, continuity of care and quality of life.

6. Observe signs and symptoms, reactions to treatments, including drugs, and report changes in the patient's physical or emotional condition to the physician.

7. Provide emotional support to patients and families in a nonjudgmental atmosphere.

8. Affirm patient/family lifestyle and involve them in decision making regarding care.

9. Provide continuity of care when patient is transferred back into the home setting.

10. Maintain complete, accurate, current patient records in accordance with Hospice record keeping procedures and requirement.

11. Participate in shift-to-shift reports, monitor and report changes in patients' condition or family situation, and participate in assessing the need for changes in the plan of care.

12. Supervise and direct care of Hospice House LPNs, NACs and volunteers scheduled on your shift.

13. Assess the LPNs and CNAs knowledge of disease and care needs of individual patients and provide education, instruction, and information as needed.

14. Assist the Clinical Nursing Supervisor in the evaluation of the CNAs skills and care provided to patients, as requested.

15. Communicate with the Clinical Nursing Supervisor about any problems, changes, or issues that may arise in providing appropriate quality care to any patient in the Hospice House.

16. Communicate any changes or problems in patients' condition, or family situation, with physician on a regular basis through telephone reports or during the physician's visit.

17. May be appointed to act in the absence of the Clinical Nursing Supervisor.

18. Attending regular Hospice House staff meetings and care conference meetings, as able.

19. Participate in training new staff and volunteers.

20. Prepare and submit reports on patient services and other daily activities as required.

21. Train in other areas of the agency including but not limited to in-home nursing visits, admissions, and triage.

22. Perform other duties as assigned.

AGENCY REQUIREMENTS

1. Demonstrate support and adherence to the agency's Mission, Vision, and Values.

2. Responsible for knowledge and compliance with agency policies, procedures, and practices.

3. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality accessing patient related information on a "need to know" basis only.

4. Reliable personal transportation, and current and valid driver's license and vehicle insurance.

5. Satisfaction of state and agency requirements for Tuberculin screening.

6. Hepatitis B immunization series, proof of series or signed declination.

7. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.

8. Must pass criminal background screening and pre-employment drug screening.

9. Ability to meet the physical demands of position (see attached physical requirements and environmental conditions).

10. Complete agency and departmental orientation and training program(s).

11. Complete all continued learning, education, policy reviews and required training by their respective deadlines.

12. Maintain licensure(s) and certification(s) required for the position.

13. Available to work as assigned/scheduled.

14. Perform other duties as assigned

Compensation

Pay Range

Starting Range - $35.00 - $55.00

Max Range - $35.50 - $73.81

Premiums:

• $2.50/hour Weeknight

• $2.25/hour Weekday

• $4.75/hour Weekend Night

• $4.00/hour On Call Standby

Note regarding the listed pay range:

At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.

Benefits

Eligibility for 401k plan (3% agency contribution and a 2% agency matching contribution starting after 1 year of continuous employment and at least 1080 hours worked within that time frame for eligible employees

Holiday pay for worked Holidays.

Sick time earned on an accrual basis (90day waiting period for new hires to utilize)
