Key and Important details: •Start your day from home on a supplied tablet •Only occasionally will you need to come to the office once you complete orientation •Full-time positions salaried work Monday -- Friday day shift •Part-time positions, hourly have a couple of different scheduling options . Make your own schedule •Per diem positions are per diem and include mileage reimbursement. We do our best to keep you in the territory that is the closest to your home or desired community to serve. Job Summary: Responsible for the total direct nursing care of all patients within established standards of care, home health, procedures, and guidelines. Primary Duties: Assesses the comprehensive nursing needs of an assigned group of patients effectively using the nursing process. Coordinates the care patient and family/significant other education process. Initiates patient start of care . Continuously plans, implements, and evaluates patient care, incorporating patient and family/significant other into the process. Participates with physician partners and other interdisciplinary team members to develop and implement patient and family/significant other plan of care. Communicates effectively all relevant information (e.g., patient information, safety issues, problems with equipment, quality issues, etc.) to the immediate supervisor, in a timely way. Communicates and documents changes in patient condition, on a timely basis, to team members, ARNP, and physician. Delivers nursing care within established standards of care, home health guidelines, policies and procedures. Functions as a patient advocate. Practices within the scope of the Registered Nurse Practice Act. Job Types: Full-time, Part-time, and Per Diem
