We are seeking an experienced Nurse Care Manager to work as a Client Care Manager and Patient Advocate. Purpose of Position

This position offers a unique opportunity to focus on patient care solutions and concierge health while collaborating with a team of nurses and therapists to find unique resolutions to our patient's needs. We not only support our patients but our teammates as well allowing for a work-life balance and flexibility.

Duties include:
  • Assess Patient needs and help build a care plan that supports a patient needs and preferences.
  • Collaborate with the HR department to secure qualified resources in the form of non-medical caregivers and nurses working hands-on with the patient
  • Oversee and manage patient care through in-person supervisory visits, and communications with medical teams and patient family to ensure high-quality care
  • Provide support and build relationships with a client's care team to promote teamwork and a positive employee experience
  • Connect patients with necessary support and build a bridge between Broad Street & the patient's medical team through Patient Advocacy
Broad Street's goal is to provide premium and customized in-home care and nursing services for those who are impacted by complex health issues or seniors who wish to remain safe and independent in the comfort of their homes. To achieve this goal, we rely on our Care Managers to develop a customized plan of care for each patient and work with our caregivers and nurses to ensure proper excitation of those care plans.

Qualifications

To qualify for the position of Care Manager an individual should meet the following:
  • Excellent written and verbal communication skills
  • The ability to work independently to proactively see a need & take necessary actions to find a solution
  • Be a strong team player and be able to step into others' roles when needed to support co-workers
  • Sufficiently assess a caregiver's skills, knowledge, and personality to ensure a mutual match when assigning caregivers to a client's case
  • Willingness & knowledge to be able to step in and provide direct care when the need arises
  • A strong sense of professionalism and accountability - As the primary point of contact and expert on a client's needs the position requires one to be responsive to ensure the client receives the needed support in a timely manner
  • Ability to work outside of normal business hours
  • RN, PT, or OT License in the State of IL Required
  • Clinical experience with specific conditions including Parkinson's Disease and other Movement Disorders, Alzheimer's Disease and other Dementias, Stroke, Cancer, and other Chronic physical or cognitive conditions (Preferred)
  • Bachelor's Degree in healthcare, social work, or related field preferred


Working Conditions/Location

This position will require travel to client/patient residences, office locations, and healthcare facilities.
