The LPN works under the direct supervision of the RN while consistently demonstrating the values and compliance with organization’s service excellence standards.

Qualifications: 

  • Graduate of an accredited school of Nursing, with a current GA LPN License. Minimum of two years of experience in hospice, oncology, med-surg or home health providing direct patient care.

  • Current CPR certification.

  • Must have valid driver's license, automobile insurance, and reliable transportation. They should effectively communicate and provide exceptional customer service to a diverse range of patients, physicians, staff, and the community. Must demonstrate exceptional documentation abilities.

  • Computer proficiency in Microsoft Office products including Outlook, and database knowledge.

Physical Demands: 

  • Position requires moderate visual concentration for patient charting/documentation and computer work.

  • Position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions.

  • The LPN requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 lbs. utilizing proper body mechanics.

Work Environment: 

  • A full-time position works a 30–40-hour workweek, rotating days, evenings, or weekends, with occasional requests to work additional days/hours/holidays as needed.

  • At times, due to patient census and acuity, this position may be required to remain on site at Hospice Inpatient Unit (HIPU) during meal breaks.

  • The Home Care position may require providing care in high-risk crime areas and unsafe situations as determined by local authorities.

  • The LPN provides care under the RN's direction and accurately documents patient/family encounters using a computer.

  • This position encounters stressful situations with patients and their families regarding end-of-life care.

  • LPNs are assigned to specific departments/regions, but may have to serve patients in other areas.

  • The Home Care LPN will be required to perform patient visits utilizing their personal vehicle and may require extensive driving.
