Summary:
Presbyterian is hiring an RN Care Manager to work at the Family Medicine Clinic at Rio Rancho High Resort. The Care Manager pro-actively manages the care for patients along a continuum by facilitating collaborative care between patient, family/support system, physicians, and other ancillary departments/members of the primary care team
QualificationsCredentials:
- Registered Nurse (RN) - State of New Mexico or Compact
- State Nursing License required.
- BLS required at time of hire.
1-3 years ambulatory experience
Education: Associate Degree in Nursing
