Summary

Working under the direction of the Northern AZ Regional Program Manager, the Justice Navigator provides reach-in services at the local county jails and Arizona Department of Corrections Rehabilitation and Re-entry (ADCRR). This position provides re-entry and community based support to individuals released from incarceration with a substance use disorder, providing support, education and resources for the purpose of successful re-engagement back to the community and the reduction of recidivism. Promotes a recovery based approach to member care that emphasizes hope, respect, self-direction and empowerment by providing peer support, personal assistance, education on community resources and other supportive services, to aid in members' recovery from mental illness and/or addiction disorders.

Essential Duties

  • Works to coordinate care with community partners, including but not limited to: Jail Liaisons with the local jails, Release Planners at ADCRR, AZCH-CCP, Care Coordinators, courts, and community health workers.
  • Provide support, transportation, and navigation services to participants in the program to ensure successful transition back into the community with appropriate resources and support.
  • Assist eligible members in enrolling with AHCCCS.
  • Identify services needed, and use HOPE's internal process for making referrals to other community stakeholders, including health homes, housing providers, employment services, and more.
  • Coordinate care with community stakeholders to assist the member in attending appointments, overcoming obstacles related to service accessibility and utilization.
  • Provide members with education on harm reduction, overdose prevention, and substance-use treatment options for the completion of goals in the program scope of work.
  • Obtain all necessary Releases of Information (ROI) to include member/potential member support system, and affiliated partners.
  • Relates own recovery process in a positive and supportive manner through coaching, role modeling and mentoring in order to assist members in developing skills to achieve their goals.
  • Appropriately uses one's own recovery journey to inform and provide support to with member to navigate and use community resources in an effective manner.
  • Provides information on service options at RBHA direct contracted agencies and fulfills referral requirements when indicated.
  • Completes documentation for behavioral health record in accordance with agency and funding source requirements. This includes tracking and reporting data to the health plan.
  • Advocates for members, as needed, to ensure delivery of necessary services.
  • Promotes HOPE, Inc. as the premier peer support organization to the behavioral health community and prospective funding sources.
  • Assists in engaging member in HOPE outpatient services as appropriate.
  • Performs other related duties in accordance with agency growth, departments, and changes.
Requirements

Qualifications
  • High School/GED
  • Graduate from a peer support training certified by the State of Arizona.
  • Strong leadership and organizational skills
  • Effective oral and written communications skills
  • Ability to implement organization-wide activities (setting objectives, developing strategies, developing policies and procedures for programming, and organizing the functions necessary to accomplish the activities)
Additional Requirements
  • (Required) Peer - Individuals must have first- hand knowledge of a behavioral health condition or substance abuse issue.
  • Must have lived experience as a person who has been incarcerated in a Jail Setting or gone through court proceedings, preferably someone who also has experience with TBI either through themselves or a family member.
  • Must be at least 21 years of age per licensing requirements.
  • Valid Arizona Driver's license, Driver's license is an essential requirement for performing duties for this position with a Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
  • 1st Aid, CPR and proof of TB within last year
