Position Overview: Physical Therapist (PT)

As a Physical Therapist at Pinnacle Home Care, you will provide essential care to patients in the comfort of their own homes. You will work closely with physicians and nursing supervisors to ensure compliance with all regulatory and clinical standards.

Key Responsibilities

  • Conduct initial and ongoing patient assessments, including OASIS documentation.
  • Create and implement customized treatment plans in collaboration with physicians, making adjustments as necessary.
  • Work alongside the interdisciplinary care team to enhance patient outcomes.
  • Educate patients and families about therapy programs and support their journey.
  • Oversee Physical Therapist Assistants every 30 days.
  • Communicate updates on patient progress and changes with physicians and care teams.
  • Document all visits and progress notes promptly on the same day of service.

Qualifications

  • Graduation from an accredited Physical Therapy program.
  • Active Physical Therapist license in Florida.
  • Current BLS certification.
  • Valid driver's license and access to a reliable, insured vehicle.
  • A minimum of one year of PT experience (home health experience is a plus).
  • Familiarity with electronic medical records is advantageous.
  • Physical ability to perform necessary tasks (standing, bending, lifting).
  • Strong self-management, communication, and decision-making skills.
