Association House of Chicago is actively hiring an Occupational Therapist to join its team in Chicago, IL.

As an Occupational Therapist, you will be part of a transition team helping individuals move from nursing home care to community-based living. This role includes assessing residents' needs, providing skill-based interventions during the transition, and continuing treatment in their new community placement. You will also collaborate with families and other providers to ensure a smooth and supportive transition process.

This role is full-time and offers a flexible schedule with the ability to work autonomously.

Job Duties:
• Review medical charts and other relevant documents for input into assessments and service plans
• Completes evaluations and assessments per program standards
• Provide occupational therapy to Class Members with wide range of developmental disabilities, motor impairments, cognitive impairments ad intellectual functioning
• Assist with discharge planning, including recommendations for adaptive equipment and support programs
• Complete all documentation accurately, completely, and in a timely manner

Job Requirements:
• Associates or bachelor’s degree from an accredited college or university in psychology, social work, or related field OR completion of a certification program in occupational therapy
• Valid Illinois OTR/L, preferred
• Experience working with individuals with developmental and physical disabilities, mental health and co-occurring disorders, required
• Bilingual Spanish/English, preferred
• Valid driver’s license with an acceptable driving record
