What You'll Do:

  • Manage patient and family caseload, including assessment and care planning
  • Communicate with attending physician and interdisciplinary team
  • Regularly assess patient needs and update care plans
  • Provide support to families during difficult times
  • Maintain accurate patient records and prepare care plans
  • Supervise and document care delivery by Home Health Aides

Qualifications:

  • Graduate from an accredited nursing program
  • Registered nurse in the state of practice
  • Current CPR and First Aid Certification
  • Knowledge of quality assessment and assurance procedures
  • Valid driver's license and proof of insurance
