Primary Duties and Responsibilities:
- Comprehensive Assessment and Service Plan Development: Conduct thorough assessments of clients with serious mental illness to identify their needs, strengths, and challenges. Develop personalized service plans (ISPs) that are tailored to each client's unique situation, incorporating a holistic approach to care.
- Coordination of Care: Serve as the primary point of contact for clients, coordinating all aspects of their care, including mental health services, medical care, housing, employment, and social support. Ensure that services are integrated and aligned with the client's goals.

- High school diploma or equivalent with 18 credit hours of post-high school education in a behavioral health field completed within the last four years and two years of full-time behavioral health work experience, or four years of behavioral health work experience.

- Experience in community-based mental health programs and case management is a plus.
- Skills and Competencies:
- SMI Knowledge: Strong understanding of serious mental illness, including symptoms, treatment options, and recovery strategies.
- Communication: Excellent verbal and written communication skills, with the ability to engage effectively with clients, families, and multidisciplinary teams.
- Crisis Management: Ability to respond quickly and effectively to crises, ensuring the safety and well-being of clients.
