Your Role as a Home Health Physical Therapist Includes:

  • Delivering home-based physical therapy to patients recovering mobility, strength, and functionality post-illness or injury
  • Collaborating with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, ensuring focused patient care
  • Conducting thorough movement assessments, documenting progress, and adjusting care plans as necessary
  • Assisting patients with therapeutic exercises and hands-on treatment to alleviate pain and enhance mobility
  • Educating patients about their care plan, therapeutic goals, and self-care practices
  • Coaching family members on the care plan and safe exercise assistance

Essential Qualifications for Home Health Physical Therapists:

  • Graduation from an accredited Physical Therapist Program and active PT license in MI
  • At least 1 year of physical therapy experience, preferably in home health
  • Current CPR certification
  • Familiarity with state and federal home health regulations
  • Excellent clinical judgment, strong interpersonal skills, and a compassionate nature
