Job Title: Licensed Practical Nurse Job Specialty: Family Health Job Duration: 48 weeks Shift: Monday through Friday, 0700-1600 Guaranteed Hours: 40 hours per week Experience: Practiced as an LVN performing hands-on patient care in the outpatient clinical setting for at least 24 months within the past 36 months License: Current, full, active, and unrestricted license as a practical or vocational nurse Certifications: Basic Life Support (AHA or ARC) Must-Have: - Graduate from an accredited community college, junior college, college, university, or vocational nursing program approved by the appropriate State agency and accredited by the Commission on Nursing Education Accreditation (CNEA) or another acceptable accreditation body outlined in the TO. Job Description: - Initiate, perform, and complete assigned duties in providing care to variable patient populations.- Complete assigned patient care based on the patient's conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report, and record relevant patient information.- Observe, identify, and respond to patient needs for care, including medication, equipment-assisted care, and patient/family education.- Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and perform treatments according to established policies/procedures.- Prepare and administer appropriate vaccinations to variable patient populations, including pediatrics according to established policies/procedures and guidelines used in the facility.- Recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.- Perform support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure.- Orient peers and ancillary personnel to unit policies and procedures, skills, and standards of practice.- Integrate healthy work environment principles that result in improved patient healthcare delivery, education of patients and their families, and employee satisfaction.- Serve as a Vocational Nurse performing patient care and nursing administrative duties for outpatients in accordance with established policies, procedures, protocols, and guidelines.- Monitor and record age-appropriate physiological measurements.- Orient new patients to the clinic/hospital environment.- Assist with patient check-ins, admissions, discharges, and transfers as directed.- Provide individualized patient teaching on appropriate topics to include preventative healthcare and health maintenance.- Prepare and position patients for diagnostic testing, examinations, treatments, and clinical procedures.- Perform appropriate portions of medical examinations, treatment, diagnostic, and clinical procedures.- Record treatments, procedures rendered, and observed effects.- Identify patient problems and accurately notify nursing team of observations.- Prepare and administer medications in accordance with scope of practice and training.- Recognize and respond to emergencies and perform basic life support measures.- Perform appropriate portions of physical examinations, including age-appropriate physical measurements.- Adhere to infection control procedures, including the use of personal protective equipment, disposal of waste, and aseptic technique.- Perform central sterile supply duties.- Ensure all equipment is properly assembled, operates, cleaned, and stored at the end of each work shift. Ensure preventive maintenance and routine cleaning are performed as directed.- Perform medical records reviews and apply appropriate protocols and guidelines to determine medical needs.- Maintain patient medical records in electronic format as well as hard copy as required.- Prepare and submit administrative reports.- Participate in training and continuing education programs for new procedures, techniques, and equipment.- Ensure a safe work environment and employee safe work habits.- Establish and maintain appropriate, professional interpersonal relationships with coworkers, families, peers, and other team members.- Have an understanding of and ability to prepare and administer sublingual, topical, inhalation medications.- Conduct and accurately document history and physical, treatments, procedures rendered, and observed effects in accordance with scope of practice and training, ensuring timeliness, accuracy of content, and signature.- Ensure patient privacy and personal dignity throughout all procedures.
