QUALIFICATIONS:

1. Minimum Bachelor's Degree

2. PA Certification in Special Education, PK-12 or experience with students in special education.

3. Maintain confidentiality as required under HIPAA and FERPA regulations

4. Demonstrated ability to develop transition plans for students in special education

5. Ability to relate to and communicate with students, parents, and staff

6. Ability to travel to, and collaborate with, local and state agencies to develop varied transition opportunities for students

7. Such alternative to the above as the Board may find appropriate and acceptable.

REPORTS TO: Special Education Supervisors/Director of Student Services

JOB GOAL: To provide district wide oversite and implementation of post-secondary transition programs for students at the Millcreek Township School District.

PERFORMANCE RESPONSIBILITIES:

  1. Following PDE requirements, coordinate, schedule, and implement Indicator 13 trainings, PAPOS Senior Surveys, Post-High School 1 year follow-up surveys, and other requirements/initiatives from PDE related to transition.
  2. Provide ongoing training to all special education teachers who support transition age students.
  3. Collaborate with IEP teams, OVR, and other agencies to provide support and services related to post-secondary transition as developed and outlined in the IEP.
  4. Develop and procure internship sites, job-shadow experiences, and work sites for students needing transition services including and not limited to developing and implementing processes for businesses, schools, teachers, and students in accordance with the most current labor laws.
  5. Coordinate with the McDowell Internship program, College Connections Counselor, Guidance, and IEP case managers for individual needs of students.
  6. Prepare for and manage a table for McDowell Trojan night for incoming 8th graders, and other Job/Career events at McDowell.
  7. Gather, maintain, and provide transition resources for special education teachers (MTSD Transition Google Site)
  8. Maintain PS Student Google site with updated information about PS for recruitment.
  9. Attend quarterly Transition Coordinator meetings at IU5.
  10. Collect OVR and Pre-ETS consents from teachers who get signed for IEPs. Enter information into student IEP Writer Demographics
  11. Procure grant funding and resources to supplement school resources.
  12. Any other duties as assigned by the Superintendent or their designee.
POSITION SPECIFICATIONS:

  1. Ability to sit, talk, and hear.
  2. Ability to stand, walk, climb, balance, stoop, kneel, crouch, or crawl and move quickly when necessary to promote student learning and/or safety.
  3. Ability to lift and/or move up to 50 pounds. Over 50 pounds requires assistance. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
SENSORY ABILITY:

1. Ability to speak clearly and distinctly.

2. Auditory acuity to be able to use telephone.

3. Visual acuity to read correspondence and computer screen.
