Seeking Autonomous, Caring Physical Therapists for Home Visits

Coastal Home Rehabilitation, a therapist-owned practice specializing in geriatric care, invites dedicated Occupational Therapists to join our team and make a meaningful impact on patients' lives. - Conduct comprehensive evaluations of patients' physical, cognitive, and functional abilities, including assessing activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Develop and implement individualized treatment plans to enhance patients' functional independence and quality of life.
- Provide hands-on therapy, one (1) patient at a time, including therapeutic interventions, adaptive techniques, and training in the use of assistive devices for improved functional outcomes.
- Educate patients and their families on strategies for safe movement, injury prevention, and maximizing independence in daily activities.
- Document patient progress and communicate with referring healthcare professionals.
- Collaborate with the rehabilitation team to ensure coordinated and effective care.
Qualifications:
- Graduate of an accredited Occupational Therapy program.
- Current state licensure as an Occupational Therapist.
- Valid driver's license and reliable transportation.
- Excellent communication and interpersonal skills.
- Strong organizational and time management skills.
- Ability to work independently and as part of a team.

Compensation: $55.00 - $65.00 per hour
