Furthermore, you will conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment, documenting any changes and adjusting care plans as necessary. REQUIREMENTS Active license to practice as an LPN Experience in long-term care, skilled nursing or rehabilitation facilities is preferred.
