This is a full-time position, Monday through Friday 8:00 a.m. to 5:00 p.m.

General Summary of Duties: Responsible for identifying the nature and urgency of the patient’s healthcare needs and determining appropriate disposition. Assess the patient’s needs, prioritize the needs, and make a referral for the proper level of care. Perform various duties following federal, state, and local regulations along with established priorities and procedures of the practice.

Major Responsibilities

  1. Take incoming text, phone calls, and portal tasks from patients and provide medical advice using provider feedback and standard protocols. Answer questions from patients on varying topics related to their healthcare. Document appropriately and timely in the EMR system.

  2. Triage patients and determine follow-up appointments. Scheduling of appointments if needed.

  3. Instruct patients on advice given by the medical provider.

  4. Create tasks and triage notes regarding lab results.

  5. Follow up on outside lab orders and results for patients. Communicate with the provider regarding lab results and document appropriately in the medical record.

  6. Meet position requirements and perform essential functions. Complete all mandatory training as required.

  7. Work collaboratively with other healthcare professionals to assist in patient care.

  8. Perform other duties as assigned.

Job Requirements

  1. State licensed practical nurse
  2. Professional customer service skills
  3. Skilled in effective written and verbal communications
  4. Active listening skills to interpret subtle indications such as tone of voice, hesitations or incomplete responses
  5. Ability to multi-task, including skills in organizing and prioritizing tasks effectively
  6. Ability to flexibly respond to changing demands
  7. Ability to work effectively as a team member with physicians and other clinical staff
