About This Career Opportunity
The RN Charge Nurse:
- Assesses, plans and implements the nursing care of residents within the skilled nursing setting with the supervision of a Registered Nurse.
- Assesses residents on admission and on a weekly basis as well as when there is a change of condition.
- Ensures continuity of care of the residents by providing direct care as well as supervising the care given by CNAs and supportive staff members.
- Follows proper infection control procedures and supervises staff in infection control procedures.
- Participates in resident and family teaching.
- Maintains the role of resident advocate with a focus on organization mission and values.

Responsibilities

- Consistently exhibits core competencies of: ECA Experience, Personal Appearance, Attendance, Resident Confidentiality, Licensing & Standards, In-service Education, Resident Compassion, and Safety.
- Assesses residents on admission and on a weekly basis as well as when there is a change of condition.
- Assesses resident condition and nursing needs and prescribes appropriate nursing action.
- Documents changes in resident's condition and nursing care in E.H.R. systems, progress notes and on the care plan.
- Notifies physicians of changes in resident condition and follows through until appropriate action is taken.
- Implements the plan of care of the resident based on assessments and goals established by the Interdisciplinary Team.
- Supervises other members of the health care team as needed to accomplish the plan of care.
- Updates plan of care as appropriate.
- Consistently involves resident and family in planning for care, treatment, and teaching needs.
- Functions as team leader for the CNAs; delegates in a manner that contributes to the excellence of care and services provided.
- Initiates problem solving and discipline in resolving performance problems of CNAs as needed.
- Demonstrates the ability to remain calm in emergency and/or stressful situations and gives everyone a feeling of confidence that the situation is under control.
- Demonstrates competence in technical and manual skills according to the scope of practice for LVNs/RNs.
- Administers medications and performs treatments according to policies and procedures and as ordered by the physician.
- Follows proper infection control procedures and supervises staff in infection control procedures.
- Demonstrates creativity in solving problems and looking at situations in new and innovative ways.
- Is a motivated, independent individual who can organize workload so that all functions are completed appropriately.
- Meets current documentation standards and policies and successfully documents in the E.H.R. system.
- Notes and carries out physician orders in a timely manner.
- Ensures confidentiality of the resident records.
- Functions as a resident advocate by protecting and ensuring resident rights.
- Interacts with residents, families, coworkers, managers and other departmental personnel in a kind, knowledgeable, professional and friendly manner.
- Gives a thorough report to oncoming shift.
- Updates 24 hour report.
- Calendars appropriately for upcoming needs.
- Follows through on shift report, and resident, family and staff concerns.
- At least 18 years of age.
- Cleared Criminal Record clearance through the Department of Social Services prior to presence in the community.
- Health screening and TB test results not older than 6 months.
- Current First Aid Training and Certifications as per Title 22.
- A current nursing license issued by the State of California is required.
- Working knowledge of computers and typing, sufficient enough to be successful with computerized Point of Care and other systems.
- Minimum of six months in long term care, home care or related field.
- Demonstrated competence in technical and manual skills according to the scope of practice for RN's/LVN's.
- Sensitivity and knowledge of working with older adults and the aging process.
- Excellent written and verbal communication skills.
- Knowledge of aging process and implications of aging in a social setting.
- Must understand health care environment and the medical context for older adults.
- Ability to communicate professionally with residents, families, peers and employees in a multi-cultural environment.
