Responsibilities

  • Provide physical therapy services and update care plans as needed
  • Perform OASIS time point assessments and discharge planning per Medicare Criteria
  • Attend staff meetings, team conferences, and required educational in-services
  • Ensure the use of the 4Ms and provide Age-Friendly Care

Qualifications

  • Current state Physical Therapy License
  • Good driving record and reliable mode of transportation
  • One year experience as a home care professional Physical Therapist preferred
  • Ability to make sound clinical judgments and assess patient needs
  • Proficiency in clinical skills and Microsoft Office
  • Ability to drive 2-4 hours per day
