Join our dedicated team as a Physical Therapist, providing essential rehabilitative care to home health patients. This role involves assisting patients with ambulation, transferring, gait training, and home exercise programs. As part of our interdisciplinary home care team, which includes RNs, PTAs, OTs, SLPs, MSWs, and Home Health/Hospice Aides, you will also deliver palliative physical therapy services to VNA Hospice patients on an as-needed basis, in alignment with their hospice care plans.

Key Responsibilities:

Physical Therapy Services

  1. Deliver physical therapy services to home health patients following a comprehensive plan of care. This includes, but is not limited to:
    • Evaluating therapeutic, rehabilitative, and functional status, and contributing to the development of the overall care plan.
    • Assessing muscle strength, mobility, gait, range of motion, and transfer capabilities.
    • Directing physical therapy treatments.
    • Educating patients, families, and caregivers on the use and maintenance of therapeutic appliances.
    • Identifying priority needs for physical therapy.
    • Communicating patients' responses to treatment or changes in their condition to their physicians as necessary.
    • Conducting assessments to evaluate the patient's home environment and identifying equipment needs.
  2. Provide physical therapy services to hospice patients using modalities such as pain relief techniques, equipment usage, and safety modifications to promote activities of daily living, maintaining dignity, independence, and quality of life.
  3. Initiate physical therapy programs and instruct other team members, patients, families, and caregivers on specific therapy programs and objectives.
  4. Participate in patient case conferences to ensure comprehensive care.
  5. Collaborate with attending physicians, home care or hospice team members, and patients' families to promote coordinated and comprehensive care.
  6. Communicate care plans effectively with all relevant parties, including physicians, team members, and patients' families.
  7. Engage in meetings, in-service training, and related activities as needed.
  8. Prepare and submit clinical and progress notes in a timely manner, in compliance with Medicare Home Health and Hospice Guidelines, licensure laws, and all relevant policies and regulations.
  9. Instruct patients, families, caregivers, and other team members in treatment regimens as necessary.
Supervision of the Physical Therapist Assistant
  1. The licensed Physical Therapist is responsible for the supervision of services provided by the Physical Therapist Assistant, tailored to the complexity and acuity of the patient’s needs.
  2. Supervision and guidance are provided following Home Health regulations.
  3. Conduct on-site visits and participate actively in patient treatment at least every seven visits or 14 days, whichever comes first.
  4. Determine and enforce the necessary level of supervision required for the Physical Therapist Assistant.
Qualifications:
  1. Bachelor's or master's degree in physical therapy from an accredited organization, such as the American Physical Therapy Association or equivalent bodies.
  2. Minimum of two years of experience as a physical therapist, with a preference for home health experience.
  3. Current Physical Therapist license from the Commonwealth of Pennsylvania or pending application.
  4. Certification in Cardio Pulmonary Resuscitation (CPR).
