Guaranteed Hours:40 hours/week, facility is allowed to call off 6x throughout a 13-week assignment.
Additional Information:CHC CRS is the only multi-specialty interdisciplinary clinic in Northern Arizona. Our 20,000 square foot clinic is designed to provide the best patient outcomes possible. Our services include 12 different pediatric specialty services, physical therapy, occupational therapy, speech therapy, registered dietician, social work, audiology and full case management. In addition, we have several other services to assist with wheelchair, medical equipment and orthotics evaluation and dispensing, hearing aids, feeding therapy and dental screenings in partnership with Northern Arizona University.
*Travelers will be booked 4 months prior to start date due to credentialing and NAH insurance. Traveler will need NPI# for billing (Application process will be through the facility)
