About this Brigham and Women's Hospital (BWH) General Inpatient (GIP) Social Worker (MSW) Position

HopeHealth is working with Brigham & Women’s Hospital (BWH) in furthering its mission to provide excellence in end-of-life care. As part of this care, we are seeking a Social Worker to work on-site at BWH in Boston, Massachusetts as part of our hospice team. This collaborative role assesses and provides psychosocial and emotional support to patients, their families and caregivers related to the patient’s terminal illness. The social worker is responsible for the implementation of standards of care for social work services and ongoing patient care services. Medical social services are provided by a qualified social worker in accordance with a plan of care. This is a part-time, 24hrs per week position working 4 days a week, 6 hours per day. This position participates in holiday coverage as needed and is eligible for benefits.

The responsibilities of the Social Worker position include:
  • Assist the physician and other team members in recognizing and understanding the social/emotional stress and/or disorder that exacerbates the symptoms related to terminal illness.
  • Assess the bereavement risk and need for to counsel related to grief issues.
  • Assess environmental safety, suicide potential/risk, and abuse or neglect risk.
  • Assess community resource, legal/financial and funeral planning needs, and ability to access services.
  • Assess caregiver’s ability to function adequately.
  • Assess special needs related to cultural diversity including communication, space, role of family members and special traditions.
  • Evaluate for long range planning and assessing ability to accept change in level of care.
  • Evaluate patient/family/caregiver response to psychosocial interventions.
  • Identify family dynamics and communication patterns.
  • Identify the developmental level of patient/family/caregivers and obstacles to learning or ability to participate in the care of the patient.
  • Identify support systems available to reduce stress and facilitate coping with end-of-life issues.
  • Identify obstacles to compliance and assisting in understanding goals and interventions.
  • Participate in IDG team meetings and consulting with and coordinating services with other disciplines. Determines patients’ level of care and planning placement.
  • Develop, revise, and implement the social work plan of care in coordination with the patient/family and the interdisciplinary team.
  • Submits accurate, relevant, and timely documentation including clinical and progress notes.
  • Assist with discharge planning.
  • Provide social services which include:
    • Short-term counseling
    • Crisis intervention
    • Assistance with Advance Directives
    • Funeral planning
    • Long-range planning/placement
    • Referrals to community resources
    • Addressing patient/family/caregiver questions and issues
    • Facilitating communication
    • Advocating for patient/family/caregiver needs
    • Anticipatory grief work
What are the qualifications for the Social Worker position?
  • Master’s Degree in Social Work from an accredited school
  • State registered independent social worker (LCSW or LICSW in Mass)
  • Three to five years of home care and/or hospice experience preferred.
  • Gerontology experience preferred.
  • Valid driver’s license with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Ability to read, analyze, and interpret care plans, physician orders and professional journals.
  • Ability to provide good written documentation in a timely manner.
  • Ability to effectively present information and respond to questions from patient/families/caregivers.
  • Must be able to proficiently use computer.
